Hospice Internship Application Form
Bloom Hospice offers internship opportunities for advanced degree level students in social work, spiritual care, and counseling. Internships are unpaid but provide opportunity for students to gain direct field experience while gathering credit towards their degree.
Bloom coordinates with each student’s program to ensure that students in our internships can complete the necessary number of hours in direct work as well as supervision. Experiences offered through Bloom internships can include interdisciplinary team collaboration, direct patient and family care, support group facilitation, individual counseling, community presentations, and applicable administrative tasks. Internship applications are accepted throughout the year. Internships begin and end based on students’ program requirements.
Bloom’s intern requirements are listed below. This list is in alignment with regulatory and agency policies.
Intern Requirements:
· Enrollment in an accredited advanced degree program in social work, spiritual care, or counseling that requires the completion of internship hours;
· Strong academic standing;
· Ability to pass a required background check;
· Compliance with the health screening and immunization requirements applicable to all Bloom employees;
· Possession of a valid driver’s license and insurance;
· Ability and willingness to travel throughout Bloom’s service area; and
· Compliance with all of Bloom Hospice’s policies and procedures.

Bloom will review your application and material to determine if you meet the intern requirements. If you meet requirements, Bloom will contact you to schedule an interview. If you are offered and accept an internship position with Bloom, you will be required to complete a health screening, orientation, and training.
